The Whorleater (Extreme) is an upscaled version of your battle with Leviathan in FFXIV. It features boosted stats for the boss, new mechanics, and different loot compared to the Hard version.
Unlocking the Extreme version of Leviathan requires the completion of a couple quests (all listed below in a step-by-step order).
First, you’ll need to complete the level 50 Main Scenario Quest “Through the Maelstrom”.
Once you complete that, an optional quest titled “Whorl of a Time” will become available from Urianger in the Waking Sands (X:6.0, Y:4.9). Progressing this quest will grant you access to the Whorleater (Extreme).

Outside the Waking Sands
After completing “Through the Maelstrom” in the MSQ, a new quest will become available from Urianger in the Waking Sands. Look for him at (X:6.0, Y:4.9) and select the quest “Whorl of a Time”.
Urianger informs you that yet another primal has been re-summoned.
This time, it is the Lord of the Whorl, Leviathan.
This version of Leviathan is seemingly fiercer than the previous incarnation the Warrior of Light had faced. Given the short amount of time it took to summon the primal once again, Urianger suspects that the Ascians have a hand in its reemergence.
Commander R’ashaht Rhiki
Since Leviathan poses a great threat to the waters near Limsa Lominsa, you’re advised to offer your aid to Maelstrom Command.
Travel to the Upper Decks and speak with R’ashaht Rhiki at (X:13.1, Y:12.8).
R’ashaht Rhiki confirms that Leviathan has indeed been summoned once more by the Sahagin. You are then instructed to report to Sergeant Styrnlona in the Moraby Drydocks at Lower La Noscea (X:24.0, Y:33.0).

Styrnlona can be found a short distance north of the Moraby Drydocks aetheryte.
She will tell you that the Whorleater, the vessel used in your first battle with Leviathan, is ready to depart for another bout with the Lord of the Whorl.
Loot from the Whorleater (Extreme)
Whorleater Extreme Coffer
Like other Extreme trials in FFXIV, the Whorleater (Extreme) will spawn a treasure coffer after defeating the primal.
Its contents will consist of items randomly chosen from a fixed pool of possible loot.
Once the coffer is opened, party members can cast lots to see who gets what.

These weapons are upgraded versions of the ones obtained from the treasure coffer rewarded to you for defeating the Whorleater Extreme. They have an item level of 100 for players level 50 and above.
The stats on these weapons are quite outdated, even for level 50.
But with their Leviathan-themed aesthetic and water effect when drawn, they are still a common choice for weapon glamours.
To obtain these weapons, you will need to trade 1 Mirror of the Whorl along the Wave version of the weapon to one of Rowena’s Representatives found in the 3 starting cities and Mor Dhona.
These NPCs can be found in each starting city’s aetheryte plaza.
Other Drops
Leviathan Card
There’s only one item that can drop directly into your inventory after defeating the Lord of the Whorl – the Leviathan card.
This is a four-star card for the Triple Triad mini-game.
